FSM policy for any in-person event will be in compliance with the mandates or regulations in place for the state or city where the event is being held.
It is our intention to keep everyone at the meeting safe and healthy.
Unless the vaccine is mandated by the state or jurisdiction where the event is held, FSM has no requirement for vaccination. That is a personal choice and must be made as a risk-benefit assessment by the individual unless it is required for health care workers in that state or city. FSM seminar attendees are health care workers and subject to whatever requirements apply to healthcare workers in that state or city. Some jurisdictions offer the option of providing a negative PCR test result within 48 hours prior to the event. If you have a personal objection or exemption, you may attend a practicum held in a state that does not have a vaccine mandate.
If masks are required indoors in the state or city where the event is held, FSM will abide by that regulation. Masks will be available for those who do not have them.
Temperature checks and COVID screening questions will be required on all days of the event for staff and attendees.
Hand sanitizer and disinfectant wipes will be available for those who wish to use them.

$1495 First Time Student – You have not taken an FSM Core course from Dr. McMakin. Livestream students will receive a 4-hour tutorial credit with a trained FSM practicum instructor.*
$749 – Current FSM Practitioner – You have attended an FSM core course from Dr. McMakin previously.
$749 Practitioner Assistant – You are an assistant to a practitioner who has attended or is currently enrolled in an FSM Core course from Dr. McMakin.
$749 Currently Enrolled Medical Student – You are currently enrolled in a medical/clinical training program, do not have an active clinical practice yet, and have not taken an FSM Core course from Dr. McMakin.
$400 Current FSM Practitioner Live-stream Only – You have attended an FSM core course from Dr. McMakin – Live-stream or Online Courses only.
FSM Instructor – You have taken the FSM Instructor Course. You have been approved by Dr. McMakin to assist in the practicum at this seminar. To request approval please email us.

In-person attendees can either call our reservation center at 1-877-685-5594 and reference block code Z5J, or use the booking link below. Keep in mind when receiving a confirmation letter from making a reservation online or through our call center, it will automatically show a resort fee. This is an IHG standard, however, since this has been waived per your contract, the hotel will absorb that fee upon check in. If anyone requires an early check-in they will need to book the night before, otherwise early check-ins can be requested, however, they are not guaranteed. Cancellations less than 48 hours prior to arrival or No Shows will result in a charge equal to one night’s room and tax. Your rooms cutoff date is 1/15/22, after this date any rooms left in your block will be released for general sale.
Guests will NOT be charged a daily resort fee of $15.00 plus tax per night. When receiving a confirmation letter from making a reservation online or through our call center, it will automatically show a resort fee. All attendee reservations must be accompanied by a first-night room deposit, or guaranteed with a major credit card of Group. The hotel will not hold any reservations unless secured by one of the above methods.
Cancellation less than 48 hours prior to arrival will result in a charge equal to one night’s room and tax, and the balance of the canceled reservation will be considered when calculating any Group attrition charges. Early departures and no-shows will result in the same charge and consequence.

In the event a guest who has requested a room within the Room Block checks out prior to the guest and reserved checkout date, Hotel will add an early checkout fee of one night’s room and tax to that guest and individual account. Guests wishing to avoid an early checkout fee should advise Hotel at or before check-in of any change in planned length of stay.
Reservations by Group and attendees against the Room Block must be received on or before Monday, January 20, 2022 (the “Cut-Off Date”). At the Cut-Off Date, Hotel will review the reservation pickup for the Event, release the unreserved rooms for general sale, and determine whether or not it can subsequently, accept reservations based on space and rate-available basis at the group rate after this date.
